GATE Referral & Permission Form (Gr. 3-8 Only)
Students in grades 3 through 8 may be referred for GATE identification by a teacher, parent or guardian, and/or school administrator. Every referred student must submit this signed parent/guardian permission form. Second-grade students do not need this form — they are tested automatically.
