When insurance under a Mutual of Omaha group term life insurance plan (voluntary and/orbasic) offered by an employer/group ceases (ex: termination, resignation, retirement), the employee has the option to continue coverage and self-pay.
A completed and signed form with initial premium payment
MUST be mailed to Mutual of Omaha within 31 days after
insurance has ceased under the group plan for your
request to be considered.
